Normal blood tests but still feel awful? What standard testing misses

Your blood tests came back normal and you still feel terrible. Both can be true. Here's what a reference range actually measures, and why 'in range' isn't the same as well.

Your blood tests came back normal and you still feel awful.

Both of those things can be true at the same time. Most women are never told how, so they're left assuming the problem must be them.


I'm Tegan, a naturopath and medical herbalist, and I work with women with endometriosis, PMOS and complex hormonal conditions. Reading blood work and functional testing is most of what I do. And the thing I hear more than anything else, usually before we've properly started, is some version of: my bloods came back fine, so I don't really know what's wrong with me.


What this usually looks like

You're tired in a way that sleeping doesn't fix. Not the tired you get after a big week. You wake up and you've already used whatever you had for the day.


You're bloated by the afternoon most days, and at some point you stopped calling it a problem, because it's been happening so long it just became how things are.


There's pain. Maybe it's your period, or maybe it's not really your period anymore. Maybe it's the days before, or ovulation pain that turns up out of nowhere, or a low ache that arrives whenever it feels like it.


Your mood has shifted in a way you don't recognise. Anxious about things that never used to bother you, or just flat, and hard to explain to anyone. You've probably wondered whether that part is even related, or whether it's stress, or whether it's just who you are now.


So at some point you did the sensible thing. You went to your doctor, you had blood tests, and they came back normal.


And you left with nothing. No explanation, nothing to actually do. Just a suggestion that it might be stress, and maybe this is what your body does now.


What I see happen next, over and over, is that you stop bringing it up. You decide you're probably making too much of it. And you get very good at working around a new normal you never agreed to.


Your doctor didn't miss anything

I want to be clear about this, because it gets said badly online.


Your doctor didn't miss anything. Your results were read correctly.


The tests you had were doing exactly what they're designed to do: Screen for disease, diagnose conditions, monitor progression and risk. That's the job of a standard panel and it's genuinely very good at it. So when it comes back clear, that matters. It means the serious things have been ruled out.


But it's a different question from the one you walked in with.


You went in asking why you feel like this. The test answered whether there's evidence of disease or risk. Those aren't the same question, and that space between them is where a lot of frustration in women's health lives.


Your GP is also working inside real constraints. There's a limited set of funded tests, there are guidelines about when each is indicated, and there's limited time. None of that is a personal failing. It's the shape of the system, and it's the same system that catches serious disease early in this country. I'd never want to trade that away.


What a reference range actually is

Once you know how these are built, most of this makes sense.

A reference range is a population measurement. A lab takes a reference group of screened, healthy people, measures them, and the range is the band that the middle 95% of them fall into. The lower limit is the 2.5th percentile and the upper limit the 97.5th.


Four things follow from that. None of them are controversial. This is standard laboratory science.

1. It describes a population, not you

Your body runs to its own set point. That isn't a wellness idea, it's a measurable thing in laboratory medicine, and there's a whole literature on how much a person varies against themselves compared with how much people vary against each other.


For many markers, the spread across the population is far wider than the range you personally live in. So your level can fall a long way from where you function best and still sit comfortably inside the range. Nothing flags. Nobody calls. But you've moved, and you can feel it.


2. Healthy people fall outside it by design

If the range only captures the middle 95%, then 5% of perfectly well people sit outside it. On any single test, a healthy person has roughly a one in twenty chance of being flagged.


That's manageable for one test. But a standard panel isn't one test, it's ten or twelve. Run the numbers and there's roughly a 40% chance that at least one marker comes back flagged in someone who is entirely well.

Which means a flag on its own doesn't tell you much. And neither does the absence of one. Both need interpreting.


3. The range depends partly on which lab you used

Laboratories establish or verify their own reference intervals using their own methods, equipment and local reference population.


I've seen the same test, on the same person, compared across two labs with meaningfully different ranges printed beside the results. Same body, same blood, slightly different definition of normal.


4. It was built to answer a different question

Reference ranges exist to identify disease, risk and progression. That's the job and they do it well. They're a threshold: is there something here that needs treating, yes or no.


They were never designed to answer is this person functioning well.

That's the gap most of my clients are sitting in.

This isn't an alternative medicine idea


Take thyroid stimulating hormone, one of the most commonly ordered tests there is.


Specialists have been arguing about where the upper limit of that range should sit for more than twenty years. Some argue it should be lower because people are being missed. Others argue it's already too low and we're treating people who don't need treating. That debate is still running in mainstream endocrinology journals.


If experts can't agree where normal begins on one of the most common tests we run, that tells you how much room sits inside that word, and how much research is still needed.


It's a bit like a pass mark. There's a real difference between scraping through and doing well, but both go down as a pass. And if the only thing anyone ever checks is whether you passed, nobody finds out you've been scraping through for years.


Not unwell enough to set anything off. Not well either. There's no box for that on a standard form.


The pattern I see most often

This isn't one client. It's a pattern I see repeatedly, so I've put it together as one story. Let's call her Libby.


She's in her early thirties, diagnosed with endometriosis about four years ago, laparoscopy confirmed, and she had genuinely good surgical care. She comes to me because the surgery dealt with the lesions and she still feels awful.


Exhausted. Bloated most days. Pain that isn't just around her period anymore. And a low-grade anxiety that crept in over the last couple of years, which she's stopped mentioning to anyone, because it feels like a separate issue and she doesn't want to seem like she's collecting complaints. She's had good surgery. She feels like she shouldn't still feel this way.


She's been thorough. Bloods more than once. Told each time that everything was normal. The last conversation ended with a suggestion that it might be stress, and maybe she should look at her sleep.

Everything she's been told is reasonable. The serious things have been ruled out. Her surgeon did what a surgeon does and did it well. Nobody has been careless with her.


But nobody has asked how the rest of her body is actually running. Her results ruled out disease and risk, correctly. They were never assessed for function. So a whole set of questions, how she's producing energy, what her nutrient status looks like, how she's handling inflammation, has simply never been asked. It isn't the question that pathway was built to answer.

What I'm actually looking at


When I look at her bloods, I'm not asking whether anything is flagged. I already know it isn't.


What I'm asking is where each result sits inside the range, and whether that's where I'd want it for someone trying to feel well, rather than someone trying to avoid disease. That's the difference between working to optimal ranges and working to normal ones.


Several of hers weren't where I'd want them. Not flagged, not abnormal. Sitting at the low or high end of what's technically acceptable.

Individually, each one would have looked like nothing.


Together, across multiple markers, they described a system that wasn't running well. And that pattern explained things her endometriosis diagnosis didn't. It explained fatigue that had nothing to do with her cycle. It explained why the low mood came on gradually rather than suddenly. It explained the bloating everyone had been treating as a food problem.


Where functional testing comes in

The bloods tell me where the pressure is. They don't tell me why it's there.

A blood test shows what's circulating right now. It can't show how her body made a hormone, how it moved through her tissue, or how well she cleared it. With endometriosis, PMOS and PMDD, that's very often exactly where the problem sits.


So the bloods showed me the dysfunction. The functional testing showed me the mechanism behind it.


And here's the part that matters most: none of those blood results were 

outside the reference range. Every one had been signed off as normal, correctly, by the standards being applied.


Without that fuller picture I'd be guessing, and wasting your time and money doing it. Instead I knew where to start, and just as importantly, what to leave alone.


The question that actually matters

It isn't are my results normal.

It's has anyone actually looked deeper.


Most women I see with endometriosis, PMOS or complex hormonal conditions have never been fully investigated. Not because anyone was careless, but because the standard pathway is built to rule out disease and identify risk, and once that's done, the pathway is finished. It was never designed to work out why you feel like this, and it doesn't have the capacity to work with you one to one on a plan that addresses it step by step.

So if you've been told everything's fine and you know it isn't, you're not being dramatic and you're not imagining it. You've been handed a fraction of the picture and asked to make sense of your whole life with it.


What looking deeper actually involves

Three things, and they work together.

  • A full health history. Your symptoms, your cycle, and what's already been ruled out tell me where to look, and just as usefully, where not to.

  • Your bloods read against optimal ranges rather than only normal ones. Where each result sits, and how the results relate to each other.

  • Functional testing, because bloods show what's circulating now, not how your body made a hormone, moved it through tissue, or cleared it. With endometriosis and PMOS, that's often where the answer is.

None of the three means much alone. Reading them together is what changes things, and it's why the women I work with get further than they did with any single test.
