What oestrogen dominance actually means and when the label gets it wrong

Oestrogen dominance is one of the most used terms in women's health and one of the least precisely applied. Here is what it actually means, when it is accurate, and when it is pointing at the wrong problem entirely.

Oestrogen dominance" has become one of those terms that spreads faster than the explanation behind it.


You hear it in a podcast. You see it in a caption. You read a symptom list and something clicks. Heavy periods. Breast tenderness. PMS that completely throws you off. Weight that won't shift no matter what you do. The label fits, so you start treating it.


The problem is that oestrogen dominance is a description, not a diagnosis. It tells you that oestrogen is acting without adequate opposition. It doesn't tell you why. And the reason behind it changes everything about what you would actually do to address it.


What oestrogen dominance actually means

The term describes a state where oestrogen's effects are not adequately balanced by progesterone. This can happen in two distinct ways.


The first is when oestrogen itself is genuinely elevated, producing more than your body can effectively use and clear.


The second is when oestrogen is within a normal range, but progesterone is low, leaving oestrogen's effects effectively unopposed. These two scenarios produce nearly identical symptoms. But they have different drivers and, critically, different treatments.


There is also a third driver that gets missed frequently: poor oestrogen metabolism. This is where oestrogen and progesterone may not be dramatically out of range, but the way the body processes and clears oestrogen produces an excess of more potent, pro-inflammatory metabolites. This is where functional testing adds a layer that standard blood tests cannot provide.


Why cycle timing matters before anything else

Does the test timing change the result?


Yes. Significantly. Before any hormone result can be meaningfully interpreted, the timing of the test matters more than almost any other variable.


Oestrogen is supposed to peak high around ovulation. That is not dominance. That is your body doing exactly what it should. The ratio between oestrogen and progesterone is also completely different across the cycle. In the follicular phase (the first half), there is essentially no progesterone. That is normal. A hormone test taken on a random day, or on day 7 of a long cycle, cannot tell you whether these hormones are actually out of balance.


For useful information, testing generally needs to cover two windows: Around day 3 to 4 for baseline follicular hormones, and 5 to 7 days after ovulation for luteal phase progesterone. If your test wasn't timed to at least one of these windows, the result needs to be treated with caution.


Is it high oestrogen, or low progesterone?

This is the question that changes the treatment approach, and the symptoms overlap so significantly that you cannot distinguish them by symptoms alone.


Heavy periods, sore breasts, mood changes in the second half of the cycle, bloating, and poor sleep around your period can all be driven by genuinely elevated oestrogen. They can equally be driven by low progesterone leaving normal oestrogen levels unopposed.


Progesterone does far more than most women are told. It has calming effects on the nervous system through a metabolite called allopregnanolone, which supports sleep and anxiety regulation. It is a natural anti-inflammatory and has immune-modulating effects. It supports thyroid function. It reduces androgen activity by inhibiting the enzyme 5-alpha reductase. It lightens the uterine lining, which is why heavy periods are often a sign that progesterone is insufficient, not that oestrogen is excessive.


When progesterone is low, you lose all of that. What remains is the stimulating, proliferative effect of oestrogen without its balancing counterpart. The symptoms are the same. The fix is entirely different.


Treating oestrogen dominance when the real issue is low progesterone means focusing on reducing oestrogen rather than building progesterone. You can spend months working on the wrong side of the equation.


Is oestrogen the cause, or just the amplifier?

What is the relationship between oestrogen and endometriosis?


In endometriosis, this distinction is clinically important. Oestrogen amplifies the disease rather than causing it. It promotes inflammation and stimulates the growth of endometrial-like tissue. But the primary driver is immune dysfunction, not oestrogen excess. Reducing oestrogen addresses one part of the picture while the underlying immune dysregulation continues.


This is part of why hormonal suppression provides meaningful relief for many women with endo and incomplete relief for others. It is also why chasing oestrogen alone, without investigating what is actually driving the hormonal imbalance, often produces partial improvement rather than resolution.


The androgen piece that most people miss

This is the layer that standard blood tests most commonly fail to capture, and it is clinically relevant in both PMOS and in some presentations of endometriosis.


In PMOS, the picture that looks like oestrogen dominance (weight that won't shift, irregular cycles, hormonal symptoms) is frequently driven by androgen excess rather than oestrogen excess. Androgens, particularly testosterone and its metabolites, can drive insulin resistance, which in turn stimulates the ovaries to produce more androgens, creating a cycle that sits entirely upstream of oestrogen.


Standard serum testosterone is often within the normal range in these cases. It is when you look at the full androgen picture, including DHEA-S, androsterone, and how testosterone is being metabolised into more potent forms like DHT, that the real pattern becomes visible. This is the level of detail a functional dried urine panel like EndoMAP is designed to show, where serum testing regularly misses it.


It is also worth noting that weight gain that won't shift, a symptom frequently attributed to oestrogen dominance, is more often driven by androgen excess in women with PMOS and Endo than by elevated oestrogen. Oestrogen actually supports insulin sensitivity and lean muscle mass. Targeting oestrogen in someone whose metabolic picture is driven by androgens won't produce the results you are hoping for.


Why some commonly recommended supplements can make things worse

A few specific examples are worth naming, because they come up frequently.


DIM - is widely recommended for oestrogen dominance. It supports oestrogen metabolism and shifts production toward safer metabolite pathways (2-OH pathway). But it reduces oestrogen broadly, not selectively. If oestrogen is not actually elevated, and the real driver is low progesterone or androgen excess, DIM addresses neither and may reduce beneficial oestrogen effects in the process.


Vitex - (Vitex agnus-castus) is commonly recommended for hormonal balance, PMS, and irregular cycles. In PMOS where LH is already elevated relative to FSH, which is a documented pattern in many PMOS presentations, Vitex can push LH higher. That is the opposite of what is needed to support ovulation. Without testing to clarify the hormonal picture, recommending Vitex to someone with PMOS is, at best, a guess.


This is not an argument against either supplement. Both have legitimate evidence behind them in appropriate clinical contexts. It is an argument for testing before recommending.


What functional testing actually shows

What does an EndoMAP test measure that standard blood tests miss?


EndoMAP goes beyond what a standard blood panel was designed to assess. Rather than measuring hormone levels in circulation (hormones still bound to carrier proteins and not yet active in tissues), it measures oestrogen metabolites, including which metabolic pathways are being used and whether the breakdown products are the safer 2-OH pathway or the more potent 4-OH and 16-OH forms. It assesses progesterone metabolites in a way that reflects actual tissue exposure. It includes androgen metabolites, showing androsterone and DHT activity that serum testing regularly misses. And it shows organic acid markers that reflect nutrient status, oxidative stress and gut microbiome activity, relevant because the estrobolome directly affects how much oestrogen is reabsorbed and recirculated.


This is why two women with identical symptoms can have very different underlying drivers, and why testing before treating is the only way to build a plan that is actually targeted.


A root cause approach

The approach that moves the needle looks different depending on what testing shows.


If low progesterone is the driver, the priority is identifying why ovulation is inadequate. Stress, thyroid function, nutritional insufficiency, and insulin resistance are all upstream causes that need addressing first.


If oestrogen excess is the real driver, the focus shifts to metabolism and clearance: liver pathways, gut health, and the specific metabolite picture that testing reveals.


If androgens are running the show, the treatment targets the androgen pathway. Herbs like Paeonia lactiflora and zinc have evidence for reducing androgen activity and supporting more regular ovulation. Addressing insulin resistance, where it is present, is often the most significant lever available.


These are not interchangeable approaches. That is exactly why the label of oestrogen dominance, without the investigation behind it, is only part of the story.
