Endometriosis is an immune dysfunction, here's why that changes the conversation
Endometriosis has long been framed as a hormone problem. But underneath the oestrogen story is an immune story, and it is the one that explains why hormonal treatment helps some women significantly and barely touches others.

The standard explanation for endometriosis usually goes something like this: oestrogen feeds it, so we suppress oestrogen, and that manages the disease.
That explanation is not wrong, however from updated research and clinical insight it is incomplete. And the part it leaves out explains why some women get significant relief from hormonal treatment while others find it only partially works, or stops working, or never fully addresses what they are living with.
Endometriosis is, at its core, a disease of immune dysfunction. Understanding that changes what is worth investigating, and what a complete approach needs to address.
What a healthy immune response looks like
In a body where the immune system is functioning as it should, endometrial-like tissue that appears outside the uterus gets cleared. The immune system identifies it as displaced and removes it before it can take hold.
In endometriosis, two distinct things fail. The clearance doesn't happen. And the same immune environment that missed the clearance actively supports the displaced tissue surviving, adhering, and invading further. These are two separate immune failures, not one hormone problem.
Research has established that women with endometriosis show evidence of immune dysregulation across multiple systems, including altered natural killer cell activity, changes in peritoneal macrophage behaviour, and an inflammatory cytokine environment that promotes lesion growth rather than clearance.
Where oestrogen actually fits
Does oestrogen cause endometriosis?
No. Oestrogen amplifies the disease. It does not cause it.
Oestrogen promotes inflammation, and endometriosis is an inflammatory condition. It stimulates the growth of endometrial-like tissue. It contributes to the pain signals associated with lesion activity. Reducing oestrogen reduces some of the fuel for that process.
But here is what makes this more complicated than simple oestrogen suppression: endometriosis tissue has the capacity to generate its own local oestrogen supply. It does this through an enzyme called aromatase, which is expressed within the lesions and converts local androgens into oestrogen on site. Endometriosis tissue also expresses higher levels of oestrogen receptor beta, which makes it more sensitive to whatever oestrogen is present.
This means that systemic hormone suppression, whether through the pill, hormonal IUDs, or other hormonal management, reduces circulating oestrogen but may not fully reach the oestrogen being produced locally within the lesions themselves. This is clinically significant. It is part of why hormonal management provides meaningful relief for many women and incomplete relief for others. The lesions can, to some degree, sustain their own oestrogen environment.
What is actually driving the immune dysfunction?
If immune dysfunction is the primary driver, the most useful long-term question is not just "how do we lower oestrogen?" It is: what is driving the immune dysregulation?
This is where the investigation needs to go deeper than hormone levels.
Systemic inflammation
Systemic inflammation is the most consistent finding in endometriosis. It is both a driver and a consequence, creating a feedback loop that hormonal management alone often cannot fully interrupt. Sources of systemic inflammation worth assessing include gut permeability, dietary patterns, chronic infections, and environmental exposures.
Gut health
The gut houses a significant portion of the immune system. Dysbiosis, meaning disruption to the gut microbiome, has been associated with altered immune responses and elevated inflammatory markers. In endometriosis, a disrupted gut environment contributes to the inflammatory load that supports lesion activity and impairs natural clearance mechanisms.
There is also a direct connection to oestrogen metabolism here. When gut bacteria are out of balance, the enzyme beta-glucuronidase becomes elevated, which causes already-processed oestrogen to be reabsorbed from the gut rather than cleared. This creates a cycle of oestrogen recirculation that systemic hormone suppression does not address.
Genetic factors
Genetic factors influence both oestrogen receptor sensitivity and immune response patterns. These are not modifiable, but they are relevant to understanding why some women develop endometriosis and why severity varies significantly even between women with similar hormonal profiles.
Environmental load
Endocrine-disrupting chemicals, compounds that interfere with hormone signalling and receptor activity at the tissue level, have been associated with higher rates of endometriosis. Reducing environmental load is part of a comprehensive approach, particularly where exposure has been significant.
Why this changes what a complete approach looks like
What does a full investigation for endometriosis include?
Hormonal management remains a valid, evidence-supported part of treating endometriosis. Nothing here is an argument against it.
But understanding endo as an immune disease means that a complete approach also addresses what is driving the immune dysregulation. That points toward gut integrity, systemic inflammation, oestrogen metabolism and clearance, and the environmental and nutritional factors that are contributing to the inflammatory environment.
For women who have been on hormonal treatment and still have significant symptoms, this is often where the untreated piece lies. Not because the hormonal treatment was wrong, but because it was working on one part of a two-part problem.
The EndoMAP panel provides information about oestrogen metabolism and androgen activity. Comprehensive stool testing provides the gut picture. Standard pathology adds inflammatory markers and thyroid function. Together, that testing builds a picture that a hormonal result alone cannot provide.
The question worth asking
If you have been managing endometriosis hormonally for any length of time and still have symptoms that are not fully resolved, the question is not whether to stop hormonal management. The question is: has anyone investigated the immune dysfunction alongside it? Has anyone looked at gut health, inflammatory markers, oestrogen metabolism pathways, and whether the full picture has been assessed?
