PCOS and Anxiety: What's really driving it
Anxiety and PCOS are closely linked, and it's not just in your head. Learn the hormonal and metabolic drivers behind it, and where to actually start.


Why PCOS and Anxiety so often come together
You've been told it's "just anxiety." Maybe you left the appointment with a prescription and a suggestion to manage your stress. What nobody looked at was your hormones, your blood sugar, or your stress physiology, the things quietly driving the way you feel.
If your anxiety seems to track with your cycle, spike when you haven't eaten, or sit alongside irregular periods, acne, or fatigue that doesn't match your life, you're not imagining the link. The anxiety is real. It also makes biological sense.
This is one of the most common patterns I see in women with PCOS. And it's rarely a coincidence.
Why I call it PMOS, not PCOS
Before we go further, a quick reframe. I refer to PCOS as PMOS: Polyendocrine Metabolic Ovarian Syndrome.
The old name puts all the attention on the ovaries and on "cysts." But what shows on an ultrasound isn't cysts in the way most people picture them. They're arrested antral follicles, follicles that simply didn't ovulate that month. PCOS is not an ovarian disease that happens to affect your metabolism. It's a cardiometabolic condition that affects your ovaries.
That distinction is important, because the same endocrine and metabolic drivers behind PMOS are the ones feeding your anxiety.
Is anxiety more common in women with PCOS?
Yes. Women with PCOS experience anxiety at significantly higher rates than women without it. It shows up across every type, and it isn't a sign of weakness or a separate problem you happen to also have.
The reason is mechanical. When your hormones and metabolism are dysregulated, your nervous system feels it. Here's what's actually happening underneath.
What's actually driving the anxiety
Low progesterone and a nervous system without its brake- In PMOS, ovulation is often irregular or absent. No ovulation means no corpus luteum, and no corpus luteum means very little progesterone. Progesterone is almost universally low in PMOS, and this is one of the clearest drivers of mood instability, poor sleep, and anxiety.
Progesterone isn't only a reproductive hormone. Its metabolite, allopregnanolone, acts on the same calming GABA pathway that many anti-anxiety medications target. When progesterone is low, that natural brake on the nervous system is missing. You can feel wired, on edge, and unable to settle, even when nothing is obviously wrong.
Blood sugar swings that feel exactly like anxiety
Most women with PMOS have some degree of insulin resistance. It's found in around 75% of lean and 95% of overweight women with the condition, independent of body weight.
When blood sugar spikes and then crashes, your body releases adrenaline and cortisol to bring it back up. That surge feels like anxiety, because physiologically it's the same response: racing heart, shakiness, a sudden sense of dread, often an hour or two after a high-carbohydrate meal or a long gap without food. It isn't in your head. It's in your bloodstream.
A stress system stuck in overdrive
Around 20 to 30% of women have what's called adrenal PMOS, driven not by insulin but by a hyperreactive stress response. In this pattern the HPA axis, your central stress system, is essentially stuck on.
On functional testing, this shows as a dysregulated cortisol curve and an exaggerated spike on waking, the body treating even small demands as threats. It's the "wired but exhausted" feeling: too activated to rest, too depleted to function. Chronically high cortisol also suppresses the brain's signal to the ovaries, which lowers progesterone further, which loosens the nervous system's brake even more. The stress and the hormones feed each other.
Put simply, the body has decided the environment isn't safe, and it responds accordingly.
Inflammation and the gut
Low-grade inflammation is common in PMOS, particularly in the lean and inflammatory type. Women with PMOS consistently show lower diversity of protective gut bacteria and more inflammatory species. When the gut wall is compromised, inflammatory byproducts can cross into the bloodstream and trigger a body-wide immune response.
That same inflammation influences the brain and mood. It's part of why gut symptoms, low mood, and anxiety so often travel together, and why supporting the gut can quietly change how you feel.
Why "just manage your anxiety" advice often falls short
Standard care tends to treat the anxiety on its own. Medication or therapy for the symptom, with no look at the progesterone, the blood sugar, or the cortisol underneath.
For some women that helps. For many it only partly works, because the driver is still there. Generic advice also doesn't distinguish between PMOS types, and what calms one woman's anxiety can be the wrong move for another. Blood-sugar-driven anxiety needs a different approach to stress-driven anxiety. Naming which one you're dealing with changes everything.
None of this means stepping away from your GP or your mental health support. The best outcomes come from working alongside that care, not instead of it. It simply means adding the layer that's usually missing: the hormonal and metabolic picture.
A root-cause approach: test, don't guess
You can't tell which driver is loudest from symptoms alone, because they overlap. This is where testing replaces guesswork.
What I look at:
Progesterone and its metabolites, to see whether you're ovulating and where your natural calming hormone sits
Cortisol mapped across the day, to see whether your stress system is over-firing, flat, or crashing
DHEA-S, the adrenal androgen that flags a stress-driven pattern
Fasting insulin, HOMA-IR, and SHBG, because fasting glucose alone can miss insulin resistance for years
Thyroid and inflammatory markers, which can mimic or magnify all of this
From there, support is matched to what your results actually show. Broadly, that tends to include:
Stabilising blood sugar first, with protein and fibre at each meal, so the adrenaline surges stop. Aggressive calorie restriction usually backfires here, because it raises cortisol and worsens the exact pattern it's meant to fix
Magnesium, which supports both insulin signalling and the stress axis, and is commonly low in PMOS
Omega-3 to lower inflammation, and inositol where insulin resistance is the main driver
Ashwagandha, used cyclically, where the picture is clearly stress-driven
Protecting sleep and building in resistance training, both of which steady blood sugar and the nervous system
Direct nervous system support, so the body can come out of survival mode
The point isn't to add ten supplements to your shelf. It's to know which driver is yours, and support it in the right order.
