Every month, your body produces a set of data. Most women are never taught how to read it.
Your cycle, from its length to the quality of your bleed to the symptoms that build in the days before, reflects what is happening in your hormonal system. Not perfectly, not as a standalone diagnosis, and not as a substitute for proper testing. But as a first layer of information, it is one you already have access to every single month without spending a cent.
Here is how to interpret what your cycle is telling you.
Cycle length: your first data point
What does cycle length tell you about your hormones?
A typical menstrual cycle runs between 24 and 35 days. A consistent cycle within that range, whether 25 days or 33 days, generally suggests that ovulation is happening on schedule and that the hormonal signalling around it is broadly intact.
A cycle shorter than 21 days can point toward a short luteal phase. The luteal phase is the time between ovulation and the period. When it is compressed, it often reflects low progesterone that is not being produced for long enough to adequately sustain the second half of the cycle.
A cycle longer than 35 days, or cycles that vary significantly month to month, often points toward irregular or absent ovulation. In PMOS, this is one of the most common presentations. Anovulatory cycles produce no progesterone, which leaves oestrogen without its balancing counterpart for the entire second half of the cycle.
An important note for women with PMOS: the report card framework changes when ovulation isn't happening consistently. Without ovulation, many of the markers below don't reflect a standard hormonal pattern. What you are reading instead is the effect of anovulation itself, which is its own set of clinical information and warrants a different kind of investigation.
Flow and bleed length: what your body is clearing
A typical menstrual bleed lasts between 2 and 7 days, with total blood loss of roughly 20 to 80ml. Variation within that range is normal.
Bleeding for longer than 7 days, soaking through a pad or tampon in under two hours, or passing clots larger than a 10 cent coin are patterns that warrant investigation. These can reflect oestrogen not being adequately cleared, anovulatory cycles where the uterine lining has built up without the balancing effect of progesterone, fibroids, adenomyosis, or thyroid dysfunction.
Light periods or a bleed that has become noticeably shorter over time can indicate oestrogen deficiency, over-suppression from hormonal contraception, or insufficient follicular development.
Spotting for several days before your period actually starts is a consistent sign of progesterone dropping too early. Progesterone maintains the uterine lining until the hormonal trigger for menstruation arrives. When it falls prematurely, the lining begins to shed before that signal is sent.
PMS severity: a clinical signal, not just inconvenience
When does PMS become a sign of hormonal imbalance?
Some premenstrual symptoms are common and are not automatically a sign of significant hormonal imbalance. Mild bloating, slightly increased sensitivity, or a small dip in energy in the few days before your period can reflect the natural hormonal shift at the end of the luteal phase.
The clinical signal is severity and duration. Mood changes significant enough to affect your relationships or your capacity to function, breast pain that limits movement, anxiety or low mood that builds across the full two weeks of the luteal phase, or bloating that is disabling rather than mild, these are not inconveniences to plan around. They are data points pointing toward a hormonal imbalance worth investigating.
PMDD, where symptoms are severe and predictably cyclical, often reflects heightened sensitivity to the normal hormonal shifts of the luteal phase rather than hormones that are dramatically out of range. The distinction matters for treatment, because the approach differs depending on whether the issue is hormone levels, hormone sensitivity, or both.
Pain patterns: where things get specific
Pain is perhaps the most information-rich part of the cycle, and the most commonly dismissed.
Pain that arrives predictably with your period and settles reliably once it ends points toward a hormonally driven cause. Prostaglandins, produced at the start of menstruation, trigger uterine contractions and are responsible for cramping. When these are elevated due to an underlying inflammatory process, pain is more severe. This is a common pattern in endometriosis.
Pain that is present for much of the month, gets progressively worse over time rather than staying stable, involves the bowel or bladder, causes pain with sex or with opening your bowels, or radiates to the leg or lower back, these patterns suggest something structural rather than purely hormonal. Endometriosis and adenomyosis are the most common causes and warrant investigation beyond cycle tracking.
The pattern of pain is the clinical clue. A period that is painful but predictable and self-limiting is a different picture from pain that has been gradually worsening over years and now affects your whole week. Both are worth discussing with a practitioner. But they are not the same conversation.
What your cycle can and cannot tell you
This framework is a first layer of information. It is not a complete diagnostic tool.
Your cycle can tell you that something is worth investigating. It cannot tell you which specific hormone is out of range, whether the driver is progesterone deficiency or androgen excess or oestrogen metabolism, or what the underlying cause of any imbalance actually is.
That is what testing is for. And the most useful thing your cycle data can do is give you a clear and specific pattern to bring to that testing, so the investigation is targeted rather than general.
